This is not an application for admission. This is the first step in the early assurance process for the Mount Union Physical Therapy Program. For the sake of disclosure please read the following items carefully:
• I understand that I am applying to interview for early assurance to the Mount Union Doctor of Physical Therapy Program. I am not applying for admission to the DPT Program at Mount Union at this time.
• I have read the DPT Early Assurance Pathway materials and understand that this pathway is a two-step process.
• I understand that if my early assurance application is accepted, my intended matriculation start term into Mount Union’s Physical Therapy (DPT) program is Fall 2027.
• I understand the academic metrics that will be used to determine my admission to the DPT Program at the University of Mount Union as stated on the program website, https://www.mountunion.edu/physical-therapy under the tab, “PT Admission.”
• I acknowledge that the DPT Program review will include accessing and reviewing my current Mount Union transcript.
• I understand that I must have a Bachelor of Science or Bachelor of Art degree from Mount Union prior to matriculation to the DPT Program at Mount Union.
• I understand that all my academic metrics used for admission to the DPT Program at the University of Mount Union, such as grade point averages, will be calculated by the PTCAS model of calculation. I have been given the opportunity to review the ‘Calculating Your PTCAS GPAs’ model at the following website: https://help.liaisonedu.com/PTCAS.
• I understand that I must disclose any academic violation(s) while enrolled in college/university level coursework (after high school) (e.g. plagiarism, academic dishonesty. etc.) at the time of application to DPT school in PTCAS. Review of these violations will occur at the time of an admission decision at a later date. Early assurance status cannot ensure that academic violation(s) while enrolled in college/university level coursework (after high school) will not result in a denial of admission at a later date.
• I understand that I must disclose any criminal offenses in a court of law at the time of application to DPT school in PTCAS. Review of these offenses will occur at the time of an admission decision at a later date. Early assurance status cannot ensure that any offenses will not result in a denial of admission at a later date.
• Any falsifications or omissions to the above will result in either my non-selection as an early assurance candidate, forfeiture of early assurance if discovered at a later date, or denial of admission at the time of application if discovered at a later date.
• I understand that I will need to undergo a criminal background check prior to matriculation if I am admitted to the DPT Program at Mount Union.
